Brand-name GLP-1 self-pay options: LillyDirect, NovoCare and TrumpRx

How brand-name self-pay works

Eli Lilly and Novo Nordisk both sell their GLP-1 weight-management medicines at set cash prices through their own pharmacy programs. You still need a valid prescription. These purchases happen outside your insurance.

The prices below were checked on the manufacturers' own pages on September 17, 2026. They change often, and several offers have end dates. The strengths are listed as they are sold. The list is not dosing guidance: your prescriber decides which medicine and strength, if any, fits you.

LillyDirect: Zepbound vial and KwikPen

Your prescriber sends the prescription to LillyDirect Pharmacy. The single-dose vial is sold only through LillyDirect self-pay. The KwikPen, a multi-dose pen, is available through LillyDirect self-pay with home delivery or pickup at Walmart Pharmacy. Needles and syringes (vial) or pen needles (KwikPen) are bought separately.

The $449 price comes from Lilly's "Self Pay Journey Program." The terms say it began February 23, 2026, and applies only if each refill is completed within 45 days of your last delivery.

To use the KwikPen self-pay savings card, the terms require that you:

• have a prescription for an approved use consistent with FDA labeling
• pay cash and not seek reimbursement from any insurer, including government programs
• not count the cost toward a deductible or out-of-pocket limit
• live in the US or Puerto Rico
• are 18 or older

The Zepbound single-dose pen is offered through LillyDirect for people using insurance. Commercially insured patients whose plan covers it may pay as little as $25 a month with Lilly's savings card, subject to monthly and annual caps.

LillyDirect: Foundayo pill

Foundayo (orforglipron) is Lilly's once-daily GLP-1 pill. It is available for self-pay through LillyDirect.

The Foundayo self-pay terms match the Zepbound terms above: a prescription for an approved use, cash-only payment, US or Puerto Rico residence, and age 18 or older.

NovoCare Pharmacy: Wegovy pen and Wegovy pill

Novo Nordisk's price guide says self-pay pricing is available with home delivery, or with a savings offer for pickup at a local pharmacy.

Under the NovoCare Pharmacy terms, you need:

• a valid prescription that includes a prescriber ID
• a prescription for an FDA-approved use in Wegovy's labeling
• to be uninsured, or to have commercial or government insurance and choose to self-pay
• to agree not to submit a claim to any insurer for the medicine

Insurance is not accepted in this program. Prescriptions are filled by CenterWell Pharmacy, CoAssist Pharmacy (doing business as NovoCare Pharmacy) or BlinkRx network pharmacies.

TrumpRx.gov

TrumpRx.gov is a federal website that lists discounted cash prices for brand-name drugs, which it calls "Presidential deals." As of September 17, 2026, it listed:

• Wegovy pill starting at $149 a month
• Wegovy pen starting at $199 a month
• Zepbound starting at $299 a month

These match the manufacturers' own starting self-pay prices shown above.

According to its FAQ, TrumpRx:

• does not sell or dispense drugs; you follow the instructions for each offer
• is free to use
• offers discounted prices only to cash-paying patients
• tells insured patients to compare the listed price with what they would pay through insurance

For some drugs it lists coupon cards you can use at local pharmacies. For others, the price is available only through the manufacturer's own website or a limited set of mail-order pharmacies.

Terms differ by product. The Wegovy pill offer on TrumpRx is a coupon you present at a pharmacy, and its terms say government beneficiaries are excluded.

Self-pay vs using insurance

• Commercial insurance that covers the drug: manufacturer savings cards can bring the copay down to as little as $25 a month, with caps. Novo's Wegovy offer caps savings at $100 a month. Lilly's Zepbound pen card caps savings at $100 a month and $1,300 a year. These copay cards exclude people enrolled in government health programs such as Medicare and Medicaid.
• Self-pay: no insurance approval is involved. The trade-off is that the payments generally do not count toward your deductible or out-of-pocket maximum, under the Lilly terms and the TrumpRx coupon terms.
• Medicare Part D: LillyDirect points eligible Part D enrollees prescribed Zepbound KwikPen or Foundayo for weight management to the Medicare GLP-1 Bridge program. See insurance, Medicare and Medicaid coverage.

Lilly and Novo Nordisk both say they can change or end these offers at any time. Check the current price before each refill.
