How to check a compounding pharmacy's license

Why check

If you are prescribed a compounded GLP-1, the pharmacy that makes it matters. FDA says it is aware of fraudulent compounded semaglutide and tirzepatide with false label information. Some labels named pharmacies that do not exist. Others named a real, licensed pharmacy that did not actually make the product.

Start with two things:

1. The pharmacy's exact name and address. Look on the vial label, the packaging or your receipt. Don't rely only on the telehealth company's brand name.
2. Which state or states license it. NABP says a legitimate online pharmacy is licensed where it is located and in the patient's state. By that standard, a Texas pharmacy shipping to you in Colorado should be licensed by both states.

These lookups were checked on September 17, 2026. Page layouts change, but the addresses below are the boards' official tools.

Texas

The Texas State Board of Pharmacy's Pharmacy Search lets you look up a pharmacy license by license number, pharmacy name, owner name, street or city. The search page also links to the board's explanation of license status codes.

Colorado

The Colorado State Board of Pharmacy is part of the Division of Professions and Occupations in the Department of Regulatory Agencies (DORA). Its homepage links to DORA's License Lookup as "Lookup a License/ Licensee."

When we loaded it, the site first asked for a "confirm you are human" security check before showing the lookup.

New York

New York registers pharmacy establishments through the State Education Department's Office of the Professions. Use its Verification Search:

• In the "Search By" menu, choose Pharmacy Establishment to search by name, or Pharmacy Establishment by Registration Number.
• When we checked, the page said its data were current as of September 16, 2026.
• It also noted that the database was under maintenance and some search results might have issues. If a pharmacy you expect to find is missing, contact the Office of the Professions before drawing conclusions.

Any other state

The National Association of Boards of Pharmacy (NABP) keeps a directory of state boards of pharmacy. Select your state to reach the board's contact details and website, where you can find its license verification tool.

Outsourcing facilities (503B)

Some compounded products come from "outsourcing facilities" registered with FDA under section 503B rather than from a traditional pharmacy. FDA keeps a searchable list of registered outsourcing facilities. It was last updated September 8, 2026, and listed 96 facilities.

For each facility, the list shows:

• registration dates
• the date of the last FDA inspection
• whether FDA issued a Form 483
• whether a recall was conducted
• any action based on the last inspection

Being on this list means the facility registered with FDA. It does not mean its products are FDA-approved.

NABP's online pharmacy tools

• Safe Site Search Tool: on Safe.pharmacy, you can enter a website address to see whether NABP has accredited it or placed it on its "Not Recommended" list.
• .pharmacy web addresses: NABP says a website ending in ".pharmacy" has been accredited by NABP, and that the domain "cannot be faked or forged" the way a logo can.

Red flags

Be cautious if a seller:

• labels products "for research purposes," "research use only" or "not for human consumption." FDA has warned companies selling semaglutide, tirzepatide, retatrutide and other drugs with those labels directly to consumers with dosing instructions.
• does not require a prescription. NABP reports that 96% of illegal online pharmacies it reviewed did not require a valid one.
• shows no pharmacy license number or state, or the name on your label does not match any board record
• charges prices far below other sources. FDA and NABP both list deals that seem too good to be true as a warning sign.
• offers "peptides" such as retatrutide. FDA says retatrutide and cagrilintide cannot be used in compounding under federal law.
• only accepts cryptocurrency or peer-to-peer payments, which NABP lists as a warning sign
• ships injectable medicine that arrives warm. FDA recommends not using it.

What a license check can't tell you

A current license shows that a pharmacy is allowed to operate. It does not show that a particular compounded drug is high quality or appropriate for you. Compounded drugs are not FDA-approved. If something looks wrong, contact your prescriber and the board that licenses the pharmacy, and report problems to FDA's MedWatch program.
